Code cannot be changed once it has been uploaded to the Programming Center and you have approved. Code updates require a new SC part number and new order.
The MCU Programming service does not include serialization or other unique code per device. note: Kinetis devices do include a customer readable unique ID.
You must be ready to upload your code when you place your order.
NXP does not provide "first articles" to confirm customer code. Customer code will be programmed into the devices exactly as the customer uploads the MCU Programming Center.
No, you must use the assigned SC part number. Within the Programming Center you can assign a customer number for you code, and will it be displayed in the verification file to help you track your software.

Most algorithms are supported but will need final approval from NXP. Approval will be requested as you upload your code.
MCU Programming is not available for Automotive Code. Some Military code is supported but needs approvals from NXP to continue, this will be requested after your code has been uploaded.
